The following are the mandatory requirements :

Name & address of your business, alongwith description of business activity.
Name & address of each business owner.
EIN number, and TIN number
Social security number,
Banking information,
North American industry classification system(NAICS) codes for products you sell.
An estimate of the annual sale in that state.

Time duration for issue of sales tax permit : +4 weeks.
